Final Forms Playbook (PDF)

We are very excited to announce that the South Whidbey School District has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.

You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.

Please review the Parent Playbook (link) to get started in FinalForms. If you require any support during the process, scroll to the page bottom and click “Use Support”. If any questions arise, please contact our Athletics Secretary, at 360-221-6808 ext 5423 or by email at kyjohnson@sw.wednet.edu.

 We are asking that ALL parents of athletes use FinalForms. 

Please register at:  https://southwhidbey-wa.finalforms.com and follow the prompts to create your account, create your students and sign your forms. Any physical forms can be dropped off at the ASB/Athletics office as usual.

ASB and Sport Fee Waiver Process:

See SWSD Policy 3510

Fee waivers will be granted to students who qualify for free or reduced-price meal benefits and have consented to share their data with other programs within the District. More information, the application to apply, or the Family Income Survey and the Consent Form in ParentSquare or a physical copy of the Consent Form can all be found on our Food Services website linked here.

ASB Fee: $20

Sports: $50 per each single sport, fourth sport is free

Students must complete online registration (Final Forms) including a physical if needed to the athletic office or scan and email it to kyjohnson@sw.wednet.edu.

South Whidbey School District offers an online payment system through called InTouch for payment using credit cards. The convenience and flexibility of InTouch will help to eliminate the need for check writing, improve efficiencies, and help reduce costs. (Payment by check or cash is still available at the school site without a convenience fee).

This service is provided by a third-party service provider, which charges the user a convenience fee:

$.50 plus 3.99% convenience fee per transaction.
