We are very excited to announce that the South Whidbey School District has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.
You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.
Please review the Parent Playbook (link) to get started in FinalForms. If you require any support during the process, scroll to the page bottom and click “Use Support”. If any questions arise, please contact our Athletics Secretary, at 360-221-6808 ext 5423 or by email at hsmith@sw.wednet.edu.

FEES DUE:
ASB Fee: $20
Sports: $50 per each single sport, fourth sport is free
We are charging for ASB and Pay to Play fees this year. Only check (payable to SWMS) and cash are acceptable forms of payments at the ASB office.
Students must complete online registration (Final Forms) including a physical if needed to the athletic office or scan and email it to hsmith@sw.wednet.edu
